FHSAA EL2 Physical Paperwork - Upload to Athletic Clearance (Make sure all sections are filled out completely. Only page 4 of this form must be uploaded. Common missing items on page 4 – student info at the top, Doctor signature, license number and info, physical completion date and/or parent & student signatures at the bottom.)

NFHS - Must create an account to complete the courses. The courses are free.  Courses need to be completed annually.
Complete video certifications listed below and upload to Athletic Clearance. (Student athlete name must be on certificate.)

• Concussion for Students
• Heat Illness
• Sudden Cardiac Arrest
• Sportsmanship

Health insurance card for student athlete needs to be uploaded to Athletic Clearance. If you do not currently hold your own health insurance, you can purchase Student Accident Insurance through the link below.

FHSAA EL3 forms need to be read and electronically signed online by the parent and student athlete on Athlete Clearance.

Statement of consent and Athletic Handbook need to be read and electronically signed on Athletic Clearance.

1. Once you create an account in the Athletic Clearance Site, you will continue to use that account year after year to renew your student's information and give consent for your child to participate.
2. Choose every sport your child is interested in playing, even if you are not sure they will.
3. All physicals must be on the FHSAA paperwork.

5. Must have the student and parent signatures in all areas required.
